What is All-on-4?

All-on-4 is a way to replace all the teeth in one jaw with a fixed bridge of new teeth held by four dental implants. The two front implants are placed upright, and the two back implants are angled. Angling the back implants lets them use the strongest bone available and avoid the nerve in the lower jaw and the sinus in the upper jaw. For many patients, this means bone grafting can be avoided.

All-on-6 works the same way with six implants. The extra implants spread the biting force more widely, which can suit patients with more bone, a strong bite, or treatment in the upper jaw. Read our guide: All-on-4 vs All-on-6: which is right for you?

Who it can help

  • Adults missing most or all of their teeth in one or both jaws
  • People whose remaining teeth are loose, broken or badly decayed and cannot be saved
  • Denture wearers tired of loose dentures, sore spots or adhesive
  • People who want fixed teeth without placing an implant for every missing tooth

Benefits of All-on-4 and All-on-6

  • Fixed teeth: they stay in place, so you can eat, speak and laugh without worrying about them moving
  • Fewer implants: a full arch on four or six implants, instead of one implant per tooth
  • Often no bone graft: angled implants can use the bone you already have
  • Temporary fixed teeth soon after surgery in many cases, so you are not left without teeth while you heal
  • Helps protect the jawbone by giving it the stimulation that dentures cannot
  • Easier to keep than dentures: no removing, soaking or adhesive

What to expect

  1. Consultation

Visit any Qualiteeth branch. Your dentist checks your teeth, gums and health, and discusses what you want from treatment.

  2. 3D CBCT scan and digital plan

A 3D scan shows the bone, nerves and sinuses. Your implants are planned on the computer for the right position and angle.

  3. Written plan and full cost

You receive a treatment plan with the timeline and the total cost before anything starts.

  4. Surgery day

Under local anaesthetic, any failing teeth are removed and the four or six implants are placed. Guided planning aims to keep the procedure efficient and comfortable.

  5. Temporary fixed teeth

In many cases, a fixed temporary bridge is fitted on the same day or within a few days, if the implants are stable.

  6. Healing

Over the next few months, the bone bonds with the implants. You eat softer foods at first and come for reviews.

  7. Final teeth

Your final, stronger bridge is made to fit your bite and smile, and fixed onto the implants.

  8. Maintenance

Regular check-ups and professional cleaning keep the implants and gums healthy for the long term.

Frequently asked questions

How much does All-on-4 cost in Malaysia?

At Qualiteeth, All-on-4 dental implants start from RM40,000 per jaw. The final cost depends on whether you need treatment on one or both jaws, extractions, bone grafting, and the material of your final teeth. All-on-6 is quoted after your consultation and 3D scan. You receive the full cost in writing before treatment starts.

What is the difference between All-on-4 and All-on-6?

Both give you a fixed full arch of teeth. All-on-4 uses four implants, with the back two angled to make the most of the bone you have, which can often avoid bone grafting. All-on-6 uses six implants to spread the biting force more widely, which some patients with more bone, or a stronger bite, may benefit from.

Can I get new teeth on the same day?

In many cases a fixed temporary set of teeth can be fitted on the day of surgery or within a few days, if the implants are stable enough. Your dentist will confirm this for your case. The final, stronger set is fitted after the implants have healed, usually a few months later.

Is All-on-4 painful?

The surgery is done under local anaesthetic, so the area is numb. Planning on a 3D scan and placing the implants with care aims to keep discomfort to a minimum. Some swelling and soreness for a few days is common afterwards, and your dentist will advise you on pain relief and soft foods.

Can I take the teeth out?

No. All-on-4 and All-on-6 teeth are fixed to the implants, so you do not remove them at home. Your dentist can remove them during check-ups for cleaning if needed.

Who is suitable for All-on-4?

Adults who have lost most or all of their teeth in one jaw, whose remaining teeth cannot be saved, or who are unhappy with loose dentures. You need enough bone for the implants and reasonable general health. Smoking and uncontrolled diabetes can affect healing.

How long does All-on-4 last?

With good cleaning and regular check-ups, the implants can last for many years. The teeth on top may wear over time and may need repair or replacement in future.
