Missing most or all of your teeth? All-on-4 and All-on-6 both give you a fixed set of new teeth on implants, but they work a little differently. This guide compares the two, explains who each suits, and covers the cost in Malaysia, for patients at our 16 branches across KL and Selangor.

What do All-on-4 and All-on-6 have in common?

Both are full-arch implant treatments. Instead of placing an implant for every missing tooth, a fixed bridge of 10 to 14 teeth is attached to a small number of implants in the jaw. The teeth stay in place: you do not take them out at night, and there is no adhesive.

Both are planned on a 3D scan, both can often be fitted with temporary fixed teeth soon after surgery, and both need the same daily cleaning and regular check-ups.

All-on-4: four implants, angled for strength

In All-on-4, the two front implants go in upright and the two back implants are tilted at an angle. Tilting the back implants lets them be longer and anchor in denser bone, while avoiding the nerve in the lower jaw and the sinus in the upper jaw.

All-on-4 may suit you if

  • You have lost bone after years of missing teeth or wearing dentures
  • You want to avoid or reduce bone grafting
  • You want fewer implants and a simpler surgery
  • Your budget is a key factor

All-on-6: six implants, more support

All-on-6 adds two more implants, spreading the biting force across six points. If one implant ever has a problem, the others share more of the load.

All-on-6 may suit you if

  • You have good bone volume along the jaw
  • You have a strong bite or grind your teeth
  • Treatment is in the upper jaw, where bone is often softer
  • You want extra support for a longer bridge

How your dentist decides

The choice is made together with you, based on:

  • Your 3D scan: how much bone you have, and where the nerves and sinuses are
  • Upper or lower jaw: upper jawbone is usually softer
  • Your bite: strong biting or grinding
  • Your health: diabetes, smoking and some medicines affect healing
  • Your goals and budget

At Qualiteeth, implants are planned on a 3D scan before surgery. Learn more about 3D computer-guided dental implants.

Looking after full-arch implants

  • Brush twice a day, including where the bridge meets the gum
  • Clean under the bridge daily with a water flosser or interdental brushes
  • Avoid smoking
  • Come for check-ups and professional cleaning as scheduled
  • Wear a night guard if you grind your teeth

Frequently asked questions

Is All-on-6 better than All-on-4?

Not always. All-on-6 spreads the biting force over more implants, which can help people with a strong bite or treatment in the upper jaw. All-on-4 needs less bone, often avoids bone grafting, and involves fewer implants. The right choice depends on your bone, bite and health.

Which costs more, All-on-4 or All-on-6?

All-on-6 usually costs more because it uses two extra implants and components. At Qualiteeth, All-on-4 starts from RM40,000 per jaw, and All-on-6 is quoted after your consultation and 3D scan.

Do I need a bone graft for All-on-4?

Often not. The back implants in All-on-4 are angled so they can use the bone that is already there. All-on-6 usually needs more bone along the jaw, so some patients need grafting first. Your 3D scan shows which applies to you.

Is All-on-4 suitable for the upper jaw?

Yes, many patients have All-on-4 in the upper jaw. Because upper jawbone is often softer, some dentists recommend more implants there. Your dentist will advise after your 3D scan.

How long does treatment take?

The implants are usually placed in one surgery. In many cases, temporary fixed teeth are fitted the same day or within a few days. The final teeth follow after a few months of healing.

Can I choose between All-on-4 and All-on-6 myself?

You can tell your dentist your preference, budget and expectations. The final recommendation depends on your bone, bite and health, and your dentist will explain the options clearly.
